Etiology

  • Progression of cervicitis from gc or chlamydia to involve uterus & fallopian tubes

Risk Factors

  • Age < 25
  • Hx STI
  • New sex partner
  • Hx sexual activity
  • Lower education level
  • Multiple sex partners

Symptoms

  • Acute lower abd/pelvic area pain, fever, malodorous smelling vaginal d/c
  • Irreg vaginal bleeding
  • Clinically more severe if tubo-ovarian abscess
  • Rarely, RUQ pain if inflammatory process spreads to peri-hepatic region (Fitz-Hugh-Curtis syndrome), between liver capsule and diaphragm

Physical Exam Findings

  • Cervical d/c
  • Cervical motion pain
  • Uterine tenderness
  • Pain on palpation adnexa
  • Rarely, RUQ pain w/peri-hepatic involvement
